You don't need to input your password in order to get the partner badge you just need to fill out the "partner badge request form - here - The information you need to give for this is your channel ID (not the whole URL) , your forum username, and the associated email address.

Nope its the previous form people could fill out. You can either enter the linked forum account via your Freedom dashboard, unless this has changed, or via the form.
