Solved How to fill out Sponsorship

I need support in what you should fill out to get sponsorships on youtube/youtube gaming.

These:
Full Name, Title, Email Address, Phone Number, Company.

1. My concern is what should I add to Title and Company.
2. Also, do any of these information show when someone sponsors you? (name, phone,etc)

Please answer these two questions.

1. your title should be Mr / Mrs / Ms / Miss / Doctor etc etc, as for the company I would either leave this blank or place N/A in it -
2. Typically only your name would be shown, but thats up to the company if they wish to make that public, all other information such as phone numbers / addresses should remain hidden from view, as its personal data.
