GFX Request Banner, Intro

I need a new youtube banner and a clean cut intro for my videos.

Banner is open for any design, just need it to look professional and visually appealing.

Intro, I was thinking something Call of Duty-esque. I'd like an animation of a bullet shot from a gun going through a player model's head, to blood spatter on a wall from the headshot, video pans off the side to reveal my gamer tag/channel name. If this is reasonable, let me know. If not, I'm open to advice and suggestions.
