I've never heard of a confirmation/approval badge. Do you mean that 'verified'-image next to your name, like on Twitter?
You must meet certain requirments wich is at least 10K subs and 1K views a day if i'm not mistaken.

You must have a "large audience"
A clean record, no copy rights etc
And recent videos.
Once you do that send a request for freedom to verify you, and if you met the requirements you will have the verification rag on your name

Correct. There's no specific "guideline". You should not think about it, or even contemplate about getting it, until you have a somewhat large, and active, fanbase (Not 3 views on each video)

As far as I have seen, generally 10k+ although I don't have anything to do with the verification stuff so don't quote me on that
